Test Code CARBANAT Carbapenem Resistance Genes, Molecular Detection, PCR

Important Note

For outreach ordering only. Not available for internal ordering.

Useful For

Detection of carbapenem-resistance genes from Enterobacterales, Acinetobacter baumannii, and Pseudomonas aeruginosa isolates by nucleic amplified acid test.
These bacteria are often resistant to all beta-lactam agents and frequently are co-resistant to multiple classes of other antimicrobial agents, leaving very few treatment options.

Specimen Requirements

Specimen Type Preferred Container/Tube Acceptable Container/Tube
Pureculture of aerobic bacterium
Sealedagar plates
Agar slant (e.g.,chocolate, tryptic soy)

Collection/Processing Instructions

Submit pure culture of organism on agar plates to keep turn-around-time to a minimum.

Clearly state the name (ID) of the organism submitted for carbapenemase testing. If the identification is not indicated, the organism will be identified, and susceptibility testing will be completed at an additional charge.

Do not send primary plates or plates with mixed cultures. Multiple organisms submitted will be rejected and facility will be asked to resubmit.

Ensure that plates are properly sealed (e.g., tape or parafilm) to prevent contamination and exposure to laboratory personnel.

Specimen Stability Information

Specimen Type Temperature Time

Agar plate or slant
Afterincubation, store and transport at room temperature
5 days

Rejection Criteria

Isolates submitted that arenot Enterobacterales, P. aeruginosa or Acinetobacter baumannii

Mixed isolation plates

Test Components

blaIMP, blaKPC, blaNDM,blaOXA-48 and blaVIM carbapenem-resistance genes.

Interpretations

Not Detected for all five of the carbapenemase targets: No carbapenemase production.

  • Note: A not detected result does not preclude the presence of other carbapenem-resistance mechanisms.
  •  If there is suspect for any other carbapenemase target the microbiology section will forward the isolate to the state lab upon client request with corresponding sensitivity report.

Detected for any of the five targets: Carbapenemase production.

*Carbapenemase producing organisms are reportable conditions in Wisconsin.
