Hematoxylin and eosin slides are usually sufficient. When appropriate, include special stained slides, unstained slides, or paraffin blocks.
Include tissue source, clinical history, copy of surgical pathology report, and other relevant diagnostic information.
Appropriate stain(s) may be performed and charged separately.
